Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a free and non-commercial open data platform and infrastructure that facilitates research on pragmatic trials. It collects and shares cleaned trial data and ratings using PRECIS-2, allowing for multiple and diverse meta-epidemiological research studies to compare treatment effects estimates across trials that have different levels of pragmatism as well as other design features.
Background
Pragmatic studies are increasingly acknowledged as providing evidence from the real world for clinical decision making. The term "pragmatic" however, is used inconsistently and its definition and measurement need further clarification. Pragmatic trials should be designed to inform clinical practice and policy decisions, not to confirm an hypothesis that is based on a clinical or physiological basis. A pragmatic trial should also strive to be as close to the real-world clinical environment as is possible, including its recruitment of participants, setting and design of the intervention, its delivery and implementation of the intervention, and the determination and analysis of outcomes and primary analysis. This is a significant distinction from explanation trials (as described by Schwartz and Lellouch1) which are designed to provide more thorough confirmation of an idea.
Truely pragmatic trials should not conceal participants or the clinicians. This could lead to a bias in the estimates of the effects of treatment. The trials that are pragmatic should also try to attract patients from a variety of health care settings, to ensure that their findings can be applied to the real world.
Finally, pragmatic trials must be focused on outcomes that matter to patients, such as the quality of life and functional recovery. This is particularly relevant for trials involving the use of invasive procedures or potential serious adverse events. In addition to these characteristics the pragmatic trial should also reduce the procedures for conducting trials and requirements for data collection to reduce costs. Additionally, pragmatic trials should seek to make their results as applicable to clinical practice as possible by ensuring that their primary analysis follows the intention-to treat approach (as described in CONSORT extensions for pragmatic trials).
Many RCTs which do not meet the criteria for pragmatism, but have features that are contrary to pragmatism have been published in journals of different types and incorrectly labeled pragmatic. This can lead to false claims of pragmatism, and the usage of the term should be made more uniform. The creation of a PRECIS-2 tool that can provide an objective and standardized assessment of pragmatic features is the first step.
Methods
In a practical trial it is the intention to inform policy or clinical decisions by demonstrating how the intervention can be implemented into routine care. This is different from explanatory trials, which test hypotheses about the causal-effect relationship in idealized settings. In this way, pragmatic trials may have lower internal validity than studies that explain and be more susceptible to biases in their design analysis, conduct, and design. Results
While the definition of pragmatism doesn't require that all clinical trials are 100% pragmatic there are benefits of including pragmatic elements in trials. These include:

It is important to understand that a pragmatic trial does not necessarily mean a low quality trial, and in fact there is an increasing number of clinical trials (as defined by MEDLINE search, but this is not specific nor sensitive) that employ the term "pragmatic" in their abstracts or titles. These terms may indicate an increased awareness of pragmatism within abstracts and titles, however it's unclear whether this is evident in content.
